New Student Registration

  • Required  Documents for Grades K-12

    To register a NEW student with Humboldt City Schools, please complete the online application at the link below. In addition to submitting the online application, the following documentation must be provided to the school.

    • 2 Proofs of Residence
    • Updated Shot Record on the Tennessee form
    • Proof of current physical exam
    • Parent or guardian must have a state-issued ID

    Optional Documents for Grades K-12

    • Social Security Card (Optional)
    • Birth Certificate - issued by a government (Optional)

    Registration is not considered complete until the online form is submitted and the following required documentation is brought to the school.
